Providing Jackson County patients the care they need to be successful and comfortable at home as long as possible is the idea behind Black River Healthcare Clinic’s Care at Home program.
Patients enrolled in the program gain access to a comprehensive care team that may consist of nurses, social workers, providers, therapists (respiratory, speech, physical, and occupational), and patient care technicians. Each patient’s care team is personalized to meet their individual health needs and equip them with the resources necessary to achieve identified health goals. The Care at Home program improves a patient’s overall continuum of care through offering services such as care planning, med management, and patient education on top of clinical care.

Chronic Care Management (CCM) is designed for patients seeking assistance with managing their chronic conditions. Patients enrolled in CCM are equipped with a comprehensive care team to support them every step of their health journey. This program focuses on putting patients in the driver’s seat by encouraging them to play an active role in their healthcare decisions and set goals around achieving a higher quality of life.
In-home services are dedicated to ensuring patients continue to receive the necessary care they need, right in their home. This program is focused on providing services that will optimize a homebound patient’s quality of life and overall health by reducing their chances of needing to be hospitalized, going to the emergency room or staying in a skilled nursing facility.
Our home & respiratory equipment services, located on our main campus directly inside of Entrance A, is a convenient option for patients seeking respiratory medical equipment.
Our palliative and end-of-life program provides patients with all the core services they may need to be comfortable and cared for at end of life including hands-on nursing care, social work care, psych/social care, advanced care planning, provider oversight, and home medical equipment such as a hospital bed and oxygen support. Our program is committed to meeting patients where they are at and delivering compassionate, personalized care throughout their final stages of life.
